PAYMENT

We greatly appreciate your business and are honored to serve you. We accept cash, check, or credit card payment. Fees must be paid before or at time of service.  24 hr  notice for cancellation required, otherwise full fee will be charged to credit card on file. We currently do not accept insurance, but are happy to provide necessary documentation if you would like to submit your own claim to your insurance company.

PoLICIES

We are required by law and our ethical guidelines to protect your confidentiality. We will not release knowledge of your treatment except in rare legal or safety situations or in order to collect payment. If you would like us to disclose any information about your treatment to any person or entity, you will need to complete and sign a release of information for each separate party.
